In order to register with the practice you must live in postal districts of either
G3 (parts), G11, G12, G13, G14, G15(parts) and G20 (parts).

Once you have decided that you want to register please contact the practice to arrange a registration appointment.
At this appointment please bring along proof of your identity and current address, as well as your NHS medical card. Don't worry if you cannot find it. You will be asked to complete a registration form and new patient questionnaire.
Please arrive 10-15 minutes before your appointment time so that the forms can be filled in and entered onto the computer system.
During your registration appointment the clinician will check your height, weight, blood pressure, urine analysis and check your medical history.
