New Patient Registration
In order to register with the practice you must be intending to live in the area for longer than 3 months from the date of registration and live in one of the following postal districts:
G3, G11, G12, G13, G14, G15, G20, G61, G62
During COVID we are not taking on new patients who already have a GP in our catchment area (See postcodes listed above).
To register with the practice you will be asked to complete a registration form (GPR), new patient questionnaire, ethnic group form and alcohol questionnaire (16 years+ only).
Please complete these forms and provide a copy of photographic ID and proof of address and then either e-mail to ggc.gp40140clinical@nhs.scot or post to the surgery at 2 Victoria Circus, Glasgow, G12 9LD.
Please do NOT come to the surgery in person with your forms, this is to reduce footfall to the surgery during the pandemic.
Once forms have been received it can take a few days for them to be processed and for you to be registered onto our system.
