COVID Vaccination Exemption Certificates: GPs and their teams cannot issue vaccination exemption certificates. Further information on the vaccination exemption certificates scheme can be found on the Scottish Government and NHS Inform websites or alternatively you can call the COVID status helpline on 0808 196 8565.

Staff Training - The surgery will be CLOSED every Tuesday between 12.30 - 13.30 for staff training. For urgent matters, there will still be a telephone service.

New Patient Registration

In order to register with the practice you must be intending to live in the area for longer than 3 months from the date of registration and live in one of the following postal districts:

G11, G12, G13, G14, G20

To register with the practice you will be asked to complete a registration form (GPR), new patient questionnaire, ethnic monitoring form and alcohol questionnaire and provide a copy of photographic ID and proof of address.

Please complete these forms and provide a copy of photographic ID and proof of address and then either come into the surgery with the requested documents or e-mail them to or post them to the surgery at 2 Victoria Circus, Glasgow, G12 9LD. 

PLEASE NOTE: If you do not provide ALL of the forms and ID as stated above, we won't be able to register you and the surgery cannot contact you to ask for the missing forms or ID, we will have to disregard your registration forms. 

Once forms have been received it can take up to 7-10 working days for them to be processed and for you to be registered onto our system. 

Please complete the documents below, completing for each person registering and come into the surgery or e-mail or post to the surgery along with a copy of photographic ID and proof of address.
